Media snobbery, pure and simple. William and Catherine have been married for almost eight years, have three children and she is STILL referred to as "Kate Middleton". The Duchess actually has three titles which could be used but no, the media stick to her maiden name.

More people would likely know who is being written about by using the name Kate Middleton than by using The Duchess of Whatever.

Personally don't care what they call her as I have no interest in anything she (or any of them) does.
